You will be involved in the planning, co-ordinating, implementing and monitoring of integrated care programmes for patients with chronic illnesses. Your responsibilities will include the development of patient health education programmes, management of patients’ and General Practitioners’ database and working with doctors and various key stakeholders to ensure the continuation of treatments after discharge. As you need to monitor performance indicators and manage all matters pertaining to integrated care processes, you should have a broad understanding of hospital functions.
